Data Protection Notice (KVKK)

[VERİ SORUMLUSU GİRİLMEDİ] (hereinafter “the Clinic”) acts as data controller under Turkish Law No. 6698 on the Protection of Personal Data (“KVKK”).

Personal Data We Process

If you submit an appointment request through this website, we process your name, telephone number, e-mail address where given, the service and physiotherapist you have asked for, your preferred date and time range, and any notes you write in the form.

If you enter information about your health in the form, that data constitutes special category personal data under Article 6 of the KVKK and is processed solely on the basis of your explicit consent.

Purposes of Processing

• Receiving and reviewing your appointment request and getting back to you
• Planning and delivering healthcare
• Meeting our legal obligations

Legal Basis

Your personal data is processed on the basis of KVKK Article 5/2-(c), “directly related to the establishment or performance of a contract”, Article 5/2-(ç), “compliance with a legal obligation”, and — for health data — Article 6/2, “explicit consent”.

Transfers

Your personal data may be transferred only to legally authorised public institutions, and only where legislation requires it. It is not shared with any other third party.

Retention Period

Your data is retained for the periods set out in the relevant legislation; where no such period is prescribed, for as long as the purpose of processing requires. At the end of that period it is erased, destroyed or anonymised.

Your Rights

Under Article 11 of the KVKK you have the right to learn whether your personal data is being processed, to request information where it has been, to learn the purpose of processing and whether the data is used in line with that purpose, to request correction where it has been processed incompletely or inaccurately, to request its erasure or destruction, and to object where the processing produces a result against you.
